I acknowledge and understand that by filling out and submitting this form, I am not guaranteed to be a contestant in any Hawaii Amateur Pankration Association LLC event(s). I am an AMATEUR FIGHTER, and have not taken/accepted any monies or compensation/gifts for any prior Pankration or Mixed Martial Arts event(s). I may choose NOT to participate in Hawaii Amateur Pankration Association LLC event(s) at any time, if chosen to be a contestant in Hawaii Amateur Pankration Association LLC event(s).

I acknowledge that I am very familiar and comfortable with the sport of Pankration/Mixed Martial Arts including its various elements of Grappling, Brazilian Jiu Jitsu, striking, kicking, takedowns, high impact throws and submissions and that I fully appreciate this is a contact sport wherein I will interact physically with potential strangers and in so doing I will be risking possible injury.

I FULLY UNDERSTAND AND ACKNOWLEDGE THAT I WILL BE ENGAGING IN A CONTACT SPORT WHICH MAY RESULT IN INJURY INCLUDING BUT NOT LIMITED TO PERMANENT DISABILITY OR DEATH. Should injury occur, I understand that I may incur severe personal, emotional, social, financial or health related losses due not only to my own actions, omissions or negligence, but also as a result of the actions, omissions or negligence of others, of the nature of this contact sport as a whole, and/or the conditions of the premises or any equipment used. I also acknowledge the possibility of other risks not known to me or not foreseeable by Hawaii Amateur Pankration Association LLC/organizers at the time of the event. I assume all risks and accept all personal responsibility for any damages that may occur as a result of my voluntary participation in any Hawaii Amateur Pankration Association LLC event.

I understand that blood work (testing of blood) are NOT required of participants in order to examine for any conditions including Hepatitis or HIV, and acknowledge that participating/engaging in this contact sport creates a risk of exposure to the blood of others and that this fact presents a risk of contacting various diseases including but not limited to Hepatitis or HIV. I release, waive and discharge ALL persons, corporations, or other legal entities involved in the production, coordination and delivery of any Hawaii Amateur Pankration Association LLC event. This release extends to Hawaii Amateur Pankration Association LLC management, all underwriters and sponsors, the facility in which event is held, all agents, employees, volunteers, and anyone assisting or acting on their behalf, and all respective members, directors, officers, advisors, employees and agents; ALL participating clubs and their affiliated clubs, their respective administrators, directors, agents, coaches and other employees or volunteers; ALL event officials, medical personal, other competitors, their parents, guardians, supervisors and coaches; ALL sponsoring companies, agencies and advertisers, all of whom referred hereafter as "releasee" - from any and all claims, demands, losses, or damages as a result of injury, including permanent injury, or death, or damage of any kind to personal property, caused or alleged to be caused in whole or in part by the actions, omissions or negligence of the releasee or otherwise.

I agree that Hawaii Amateur Pankration Association LLC shall have the rights, and may grant others the right, to disseminate, display, reproduce, use, print, publish, and make any other uses of my name, voice, persona, and likeness in connection with advertising, and publicizing Hawaii Amateur Pankration Association LLC event and future events. Hawaii Amateur Pankration Association LLC also has the right to use and authorize others to use the same in connection with the broadcast of all or part of any Hawaii Amateur Pankration Association LLC event including all forms of media of advertising and publicity including print media, home video cassettes, pay per view, DVD and the internet.
